This panel addresses iron-deficiency anaemia — the most prevalent nutritional deficiency globally, affecting an estimated 1.2 billion people. Women of reproductive age are disproportionately affected due to menstrual blood loss, with prevalence rates of 10-20% in European women aged 15-49. The complete blood count (morfologia) provides the foundational haematological data: haemoglobin concentration defines anaemia (below 12 g/dL in women, below 13 g/dL in men), while mean corpuscular volume (MCV) classifies it by morphology. Iron-deficiency anaemia is characteristically microcytic (low MCV) and hypochromic (low MCH), though early or mild cases may present with normal red cell indices. Serum iron measures the circulating iron available for erythropoiesis, but its diagnostic utility is limited by significant diurnal variation and acute-phase reactant behaviour — iron drops during infection and inflammation regardless of actual iron stores. Ferritin is the most diagnostically reliable marker of iron stores. A ferritin below 15 ng/mL is virtually diagnostic of iron deficiency. However, ferritin is also an acute-phase protein that rises during inflammation, infection, and liver disease. In the presence of concurrent inflammatory conditions, a higher ferritin cutoff (below 30-50 ng/mL) is used to identify functional iron deficiency. Importantly, ferritin depletion precedes anaemia by months — a low ferritin with normal haemoglobin indicates iron depletion without frank anaemia, a state associated with fatigue, reduced exercise tolerance, hair loss, and impaired cognitive function. This pre-anaemic iron deficiency responds well to oral supplementation.
