Blepharoplasty — Eyelid Surgery

Blepharoplasty at ILK is performed by three experienced plastic surgeons — prim. MUDr. Vladimír Mařík, MUDr. Pavel Kurial, and MUDr. Jan Kasper — under local anaesthesia as an outpatient procedure. Upper blepharoplasty removes the excess skin and fat pads that hood the upper eyelid, restoring the eyelid crease and improving the visual field in functional cases. Lower blepharoplasty addresses under-eye bags by removing or repositioning the herniated fat and tightening the lower lid skin. Combined upper and lower blepharoplasty delivers a comprehensive periorbital rejuvenation. The procedure takes approximately 1 hour; sutures are removed within 10 days and bruising typically resolves within 2–3 weeks.

The procedure is performed under local anaesthesia on an outpatient basis — no overnight stay is required. Upper blepharoplasty involves a precisely marked incision placed in the natural eyelid crease, through which redundant skin and the medial and central fat pads responsible for puffiness are removed or repositioned. The incision is closed with fine sutures that are concealed within the crease and become virtually invisible once healed. In patients where excess skin is encroaching on the visual field (a functional indication), upper blepharoplasty also restores peripheral vision. Lower blepharoplasty addresses the three compartments of lower eyelid fat herniation (medial, central, and lateral fat pads) that produce under-eye bags. The surgeon accesses these compartments either transcutaneously (through a skin incision just below the lash line, suitable when skin removal is also needed) or transconjunctivally (through the conjunctival inner surface, leaving no visible external scar, suitable for fat removal only). Skin laxity in the lower lid may be addressed by adding a skin-pinch excision or a canthopexy to improve lid tone. Combined upper and lower blepharoplasty (from 25,000 Kč) addresses both compartments in the same anaesthetic session, taking approximately 1.5–2 hours. Sutures are removed within 10 days post-operatively. Bruising and swelling are normal post-surgical findings that peak at 2–4 days and progressively resolve over 2–3 weeks; cold compresses and head elevation are recommended. The final cosmetic result is fully visible at 6–8 weeks when all swelling has resolved.
