The incision is closed with fine sutures that are concealed within the crease and become virtually invisible once healed. In patients where excess skin is encroaching on the visual field (a functional indication), upper blepharoplasty also restores peripheral vision. Lower blepharoplasty addresses the three compartments of lower eyelid fat herniation (medial, central, and lateral fat pads) that produce under-eye bags. The surgeon accesses these compartments either transcutaneously (through a skin incision just below the lash line, suitable when skin removal is also needed) or transconjunctivally (through the conjunctival inner surface, leaving no visible external scar, suitable for fat removal only).
