For patients with concurrent true ptosis (levator muscle dysfunction causing the upper lid margin to fall across the pupil), ptosis repair can be incorporated into the same procedure. Lower blepharoplasty addresses under-eye fat bags and skin laxity through either a transconjunctival approach (incision inside the lower lid, no external scar — preferred when skin laxity is minimal) or a subciliary approach (incision just below the lash line for cases requiring skin removal).
