The lymphatic system is a one-way drainage network of vessels and nodes that collects excess interstitial fluid, proteins, and immune cells from the peripheral tissues and returns them to the venous circulation. When the lymphatic load exceeds the system's transport capacity — due to surgery, injury, inflammation, or primary lymphatic insufficiency — fluid accumulates as visible swelling (edema or lymphedema). Manual lymphatic drainage (MLD), developed by Danish therapists Emil and Estrid Vodder in the 1930s and subsequently refined by Foldi, Casley-Smith, and others, addresses this by mechanically augmenting lymphatic propulsion. The technique uses very light pressure (20–40 mmHg — lighter than standard massage) and specific repetitive directional strokes applied in a precise anatomical sequence. The direction of strokes follows the lymphatic watershed anatomy, moving fluid toward the proximal lymph node groups where the lymph re-enters the bloodstream. The sequence begins at the terminus (the neck and axillary nodes) and works distally, always 'clearing space' in the proximal vessels before directing more fluid toward them.
