Eyelid Surgery
Duration
1 hour
Blepharoplasty at EsteticUsti corrects drooping upper eyelid skin and herniated lower eyelid fat that makes patients appear tired, older, or reduces the visual field. The one-hour procedure is performed under local anaesthesia (general on request), with incisions placed precisely in the natural eyelid crease for upper lids or just below the lash line for lower lids — resulting in scars that become virtually invisible within weeks. Excess skin and, where present, prolapsed fat pads are removed or repositioned. Sutures are removed at days 7–8. Patients may return to work within 5 days and apply makeup after day 10.
Blepharoplasty is one of the most common aesthetic surgical procedures globally, addressing two distinct anatomical changes that affect the periorbital region with age. Upper lid blepharoplasty corrects dermatochalasis — the redundant, overhanging skin of the upper eyelid caused by loss of elasticity, gravitational descent, and the brow ptosis that loads additional skin onto the lid. In severe cases, this excess skin encroaches on the superior visual field (pseudoptosis), creating a functional as well as aesthetic problem. Lower lid blepharoplasty addresses prolapse of the three periorbital fat compartments (medial, central, and lateral) through weakened orbital septum, producing the characteristic lower eyelid bags. At EsteticUsti, upper blepharoplasty incisions are placed in the natural supratarsal crease (where the lid folds when open), concealing the scar within the crease when the eyes are open. For lower lids, a subciliary approach places the incision 1–2 mm below the lash line; alternatively, a transconjunctival approach (incision inside the lower lid) leaves no external scar, making it the preferred technique when only fat redistribution is needed without skin removal. For fat prolapse correction, the herniated orbital fat pads may be excised or — increasingly favoured — transposed into the tear trough depression, simultaneously correcting the lower lid bag and the infraorbital hollowness without creating a skeletonised, operated appearance. The procedure is performed under local anaesthesia for most patients; general anaesthesia is available for anxious patients or combined upper-lower procedures requiring longer operating time. Pre-operatively, patients should arrange medical clearance and gynaecological examination, stop aspirin one month prior, and plan 5 days away from work. Sutures are removed at days 7–8; makeup is permitted from day 10; sports and vigorous activity from week 3. Full resolution of swelling, revealing final results, takes 4–6 weeks.
