The MiXto SX is a fractional CO2 laser manufactured by Lasering (Italy), designed around the principle of fractional photothermolysis: the CO2 beam (wavelength 10,600 nm) is divided into a precise grid of micro-beams, each creating a narrow column of thermally ablated tissue (a micro-ablation zone or MAZ) through the epidermis and into the upper dermis. Between the ablated micro-columns, islands of intact, uninjured skin remain. These islands serve as a cellular reservoir — keratinocytes and fibroblasts migrate from the intact zones into the ablated columns, re-epithelialising the surface rapidly while simultaneously initiating a healing cascade in the dermal zone: fibroblasts are activated to produce new collagen and elastin, and inflammatory mediators drive tissue remodelling that continues for up to six months. The 'fractional' design is the key differentiator from older fully ablative CO2 lasers: because only a fraction of the skin surface is ablated per session (typically 20–40% of the treatment area, adjustable), the healing time is drastically shorter. Post-treatment, the skin should be kept clean and moisturised with a prescribed healing cream; sun avoidance and SPF use are essential.
