The MiXto SX is a fractional CO2 laser manufactured by Lasering (Italy), designed around the principle of fractional photothermolysis: the CO2 beam (wavelength 10,600 nm) is divided into a precise grid of micro-beams, each creating a narrow column of thermally ablated tissue (a micro-ablation zone or MAZ) through the epidermis and into the upper dermis. Between the ablated micro-columns, islands of intact, uninjured skin remain. These islands serve as a cellular reservoir — keratinocytes and fibroblasts migrate from the intact zones into the ablated columns, re-epithelialising the surface rapidly while simultaneously initiating a healing cascade in the dermal zone: fibroblasts are activated to produce new collagen and elastin, and inflammatory mediators drive tissue remodelling that continues for up to six months. The 'fractional' design is the key differentiator from older fully ablative CO2 lasers: because only a fraction of the skin surface is ablated per session (typically 20–40% of the treatment area, adjustable), the healing time is drastically shorter. A fully ablative CO2 treatment may require two weeks or more of recovery; a fractional treatment of similar depth requires only four to seven days. This also allows the treatment intensity to be calibrated — more conservative settings for patients with limited downtime tolerance, more aggressive settings for patients with significant acne scarring or deep wrinkles. The laser handpiece is passed systematically across the treatment zone — full-face treatment takes approximately thirty minutes. During the session patients experience a sensation of heat and brief stinging at each pulse, described as similar to hot pinprick. Immediately after, the skin appears pink-red with fine dot-like marks visible where each micro-beam was delivered; a warm, sunburn-like sensation persists for thirty minutes to three hours. Post-treatment, the skin should be kept clean and moisturised with a prescribed healing cream; sun avoidance and SPF use are essential. Surface crusting and mild scabbing over the micro-ablation dots is normal and resolves within four to seven days as the re-epithelialisation completes. Underlying collagen remodelling and tightening continue to develop over three to six months after each session, meaning the final result is not fully apparent until several months post-treatment. For significant wrinkles or acne scarring, a second session may be planned with an eight-week interval. The MiXto SX is indicated for: fine-to-moderate wrinkles, mild-to-moderate skin laxity, sun spot and age spot reduction, post-acne scarring (including ice-pick and rolling scars), enlarged pore appearance and overall textural irregularity. The treatment can be applied to the face, neck, décolletage and hands. The immediate tightening effect visible at session completion is due to collagen fibre contraction on heat exposure; the sustained tightening develops as new collagen matures.
