Unlike hyaluronic acid fillers that add volume mechanically, polynucleotides work biologically — they do not fill or volumise but instead restore the functional quality of skin that has thinned or lost resilience with age. Polynucleotides are long-chain DNA molecules — specifically PDRN (polydeoxyribonucleotide) fragments — extracted from the milt (semen) of wild Pacific salmon (Oncorhynchus keta). The choice of salmon milt is not arbitrary: the DNA sequence similarity between salmon and human cells is sufficiently close that the body does not mount an immune response, while the production and purification process ensures complete removal of proteins that could cause sensitisation. Polynucleotide biostimulation is a younger generation of skin regenerative treatment, with significant supporting clinical data particularly from Korean and Italian dermatology journals. The mechanism of action differs fundamentally from both fillers and conventional mesotherapy cocktails. PDRN fragments bind to A2A adenosine receptors on fibroblasts and keratinocytes, activating the VEGF (vascular endothelial growth factor) pathway and increasing local cell proliferation, collagen type I and III synthesis, and hyaluronic acid production within the extracellular matrix. Simultaneously, they modulate the inflammatory cascade — reducing pro-inflammatory cytokines — and scavenge free radicals, providing antioxidant protection to skin cells under oxidative stress. The net result is skin that is more hydrated at the cellular level (not superficially plumped), more structurally dense, firmer in contour, and more uniformly pigmented. At Medest, MUDr. Vojtková uses polynucleotide biostimulation primarily for three zones: the eye area (periorbital skin, crow's feet, fine lines, dark circles caused by thin skin or dehydration), the face (overall skin quality, fine rhytides, early laxity), and the neck and décolleté (crepe-like texture, horizontal neck lines, sun damage). The treatment is also suitable for the backs of hands and any other area showing quality-of-ageing changes rather than volume loss. Sessions take 30–45 minutes. An anaesthetic cream is applied before injection; the polynucleotide solution is delivered with very fine needles or a cannula into the mid-dermis in a papule technique. Mild redness and small papules at injection sites resolve within 24–48 hours. Patients must avoid solarium, sauna, and extreme temperature changes for one week post-treatment and should not touch the treated area for six hours. Smoking significantly impairs skin regeneration and should be avoided. The recommended treatment course is two to three sessions spaced two to four weeks apart; because results are driven by biological remodelling they develop gradually over the weeks following each session — the full programme benefit is visible six to eight weeks after the final session.
