Pigmentation is one of the most challenging conditions to treat in aesthetic medicine, particularly in Asian and mixed-ethnicity populations where the risk of post-inflammatory hyperpigmentation (PIH) is elevated and conditions like melasma often involve both epidermal and deep dermal components. The 755 nm Alexandrite picosecond system shatters melanin particles into finer fragments via photoacoustic pressure, triggering their clearance through the lymphatic system with minimal thermal injury and therefore lower PIH risk compared to nanosecond predecessors. Deep dermal pigmentation — Hori's naevus, naevus of Ota, and deep inflammatory marks — requires penetrating laser energy at longer wavelengths. The 1064 nm Nd:YAG reaches the dermis effectively and can also be used in low-fluence toning modes (often called laser toning or carbon laser peel protocols) that gradually reduce diffuse pigmentation over a series of sessions without the aggressive single-session approach that triggers rebound darkening. Melasma at Ensoul is addressed with a combination approach. Because melasma is deeply linked to UV exposure, hormonal influences, and vascular factors, no single laser modality resolves it durably in isolation. The clinic's protocol typically combines low-fluence Nd:YAG toning, pulsed radiofrequency microneedling (which delivers dermal remodelling without surface melanin stimulation), topical depigmenting agents, and strict photoprotection advice. This reduces the risk of the paradoxical darkening that aggressive laser monotherapy often causes.
