Upper blepharoplasty at Dr. Yael Halaas's New York City practice addresses the common concerns of excess or hooding upper eyelid skin, prominent fat pads, and heaviness of the brow–eyelid complex that develop as the periocular tissues lose elasticity and volume with age. Dr. Halaas places incisions within the natural supratarsal crease of the upper eyelid — the existing skin fold — so that resulting scars are concealed when the eye is open and fade to near-imperceptible lines over time. Through these incisions, redundant skin and any excess orbicularis oculi muscle are carefully excised, and pre-aponeurotic fat pads are either removed or redistributed to avoid the hollowed look that over-aggressive excision can produce. The procedure is typically performed under local anaesthesia with optional oral sedation (Valium) in the office setting, though general anaesthesia is available for patients who prefer it. Duration is approximately one to one and a half hours for the upper lids alone. Recovery involves mild swelling and bruising resolving over one to two weeks, with most patients comfortable returning to work within seven to ten days.
