Dr Leo employs a composite rhinoplasty technique designed specifically for Asian nasal anatomy, which typically presents with a lower bridge, wider alar base, and bulbous, fat-heavy nasal tip. The technique combines: a silicone implant to augment the nasal dorsum (bridge); tip plasty to debulk excess fat and cartilage at the nasal tip; harvested ear cartilage grafted at the tip to add projection and definition; and alloplastic material for correcting upturned or over-rotated noses. Alarplasty (reduction of the nostril flare) is incorporated where indicated. Two surgical approaches are offered. Closed (endonasal) rhinoplasty places all incisions inside the nostrils — no external scar, shorter operative time, and faster recovery. It is the most common approach at the clinic. Open rhinoplasty uses a 4–5mm incision at the columella (the strip between the nostrils), providing better visibility for complex tip work, revision cases, or significant structural changes. The columellar scar typically fades to near-invisibility over several months. Both approaches are performed as day surgery under sedation, with an operative duration of approximately 2 hours. Columellar sutures (open technique) are removed at 1 week; alar sutures at 10–14 days. The procedure also addresses functional concerns such as deviated septum and breathing difficulties. Final rhinoplasty results take 6–12 months as swelling gradually subsides.
