Surgical upper and lower eyelid rejuvenation to remove or reposition excess skin, muscle, and fat. Upper blepharoplasty removes hooding and excess skin; lower blepharoplasty addresses bulging fat pads and under-eye hollowing via transconjunctival (scarless) or external lash-line incision. Dr. Kolker performs blepharoplasty with a nuanced approach that distinguishes between the structural concerns of the upper eyelid versus the lower eyelid, and prioritises natural-looking rejuvenation over an overdone result. Upper blepharoplasty addresses excess or redundant upper eyelid skin (dermatochalasis) — the heaviness and hooding that blunts the upper eyelid aperture and can eventually impair the superior visual field. The procedure removes a measured ellipse of skin and, where appropriate, a conservative amount of pre-tarsal fat through a well-camouflaged incision in the natural eyelid crease. The result is a refreshed, alert appearance rather than a wide-eyed or hollow look. Lower blepharoplasty addresses the prolapsed orbital fat pads responsible for under-eye puffiness, as well as the tear trough hollowing that frequently coexists beneath them. In patients where fat excess is the primary concern without significant skin laxity, the transconjunctival approach — an incision on the inner surface of the eyelid — provides direct fat access with no visible external scar. Fat may be excised or repositioned into the tear trough to reduce hollowing. When lower eyelid skin laxity or fine wrinkling also requires correction, a lash-line incision is used with conservative skin excision. Canthopexy (tightening of the outer corner of the eye) and septal cinching (reinforcing the orbital septum) are adjuncts available to improve structural support and reduce lower lid rounding. The Lumenis UltraPulse CO2 laser or Fraxel Dual is frequently used as an adjunct pass to resurface lower eyelid skin texture, addressing crepiness that surgery alone cannot correct. Procedures are performed in Dr. Kolker's accredited on-site surgery suite under general anesthesia or deep sedation. Most patients return to social activities within ten to fourteen days.
