This panel provides the minimum set of hormones needed to evaluate male reproductive and metabolic health comprehensively. Total testosterone is the headline marker, but its clinical interpretation depends on SHBG (sex hormone-binding globulin), which determines how much testosterone is biologically available. A man with high SHBG may have low free testosterone despite normal total testosterone — explaining symptoms of hypogonadism that routine testing misses. LH (luteinising hormone) and FSH (follicle-stimulating hormone) are pituitary hormones that drive testicular function. Their levels distinguish primary hypogonadism (testicular failure — high LH/FSH, low testosterone) from secondary hypogonadism (pituitary/hypothalamic — low LH/FSH, low testosterone). This distinction is clinically critical because secondary hypogonadism may be caused by pituitary tumours, hyperprolactinaemia, or medications, requiring different investigation and treatment. FSH specifically reflects spermatogenesis — elevated FSH with low sperm count indicates testicular damage, while normal FSH with low count suggests obstructive causes. This makes the panel relevant for fertility assessment. TSH is included because thyroid dysfunction mimics many symptoms of testosterone deficiency: fatigue, weight changes, mood disturbance, and reduced libido. Hypothyroidism also directly impairs testosterone production. Age-related testosterone decline (1–2% per year from age 30) is a normal process, but symptoms of deficiency warrant investigation when they significantly impact quality of life.
