The SmartXide DOT (DEKA Medical Lasers, Italy) is a 10,600 nm fractional CO2 platform that applies DOT Therapy — a fractional ablation pattern in which laser energy is delivered in a matrix of microscopic dots, each creating a precise ablative channel through the epidermis and into the papillary or reticular dermis, with thermally coagulated margins. The spacing between dots preserves intact skin bridges across the entire treated area. These bridge zones contain stem cells and dermal fibroblasts that drive accelerated wound healing and collagen neogenesis, closing the microscopic channels within days while remodelling the surrounding tissue for months afterward. What distinguishes the SmartXide DOT from earlier fractional devices is programmable density control: the physician can adjust the spacing of the dot matrix per cm², effectively controlling the ratio of ablated to unablated tissue and therefore calibrating the balance between efficacy and recovery time. Higher density settings are used for severe photodamage, deep acne scars, or significant laxity; lower density settings suit patients who need minimal downtime or have more sensitive skin. At DermaStar, the device is used for a broad range of indications. Full-face fractional resurfacing (7,500 Kč) is the primary treatment for comprehensive photodamage reversal — wrinkle reduction, skin tightening, improved texture and tone, and reduction of solar lentigines. Smaller area treatments are available at lower price points for targeted concerns: periorbital wrinkles (under and around eyes), isolated pigmentation on the forehead or cheeks, post-acne scarring, stretch marks on the body, and acne scarring on the back. Pre-treatment: EMLA topical anaesthetic cream is applied approximately 30–60 minutes before the session and is included in the price. This is the primary anaesthetic; no sedation or IV anaesthesia is required for CO2 fractional resurfacing at the densities used. During the treatment: the session duration varies from 15 minutes for a small zone to approximately 30–45 minutes for a full face. The laser handpiece passes systematically over the treatment area, delivering programmed pulses. Patients feel warmth and mild pressure; the topical anaesthetic substantially reduces discomfort. Post-treatment recovery: immediately after the session, the skin appears red and feels warm, similar to moderate sunburn. Days 3–4, light peeling or flaking begins as the superficial ablated columns exfoliate. By days 7–10, the skin has re-epithelialised completely. Makeup can be applied from day 3 in most cases. The remodelling phase — collagen production, continued tightening — continues for 3–6 months. Contraindications: active use of isotretinoin (Roaccutane) within the past 6 months, blood-thinning medications, pregnancy, active herpes labialis (prophylactic antivirals may be prescribed by the physician), and Fitzpatrick skin types V–VI (higher risk of post-inflammatory hyperpigmentation — to be assessed individually). Sun exposure must be avoided for several weeks after treatment; SPF 50 is essential.
