Pneumatic intermittent compression therapy (PICT) — also known as presotherapy in the European clinical tradition — uses an inflatable garment connected to a pump unit that cycles through a programmed pressure sequence. In the Lympha-Press system used at DermaEsthetic, each section of the garment contains an independent air chamber, allowing the pump to create a sequential peristaltic wave: chambers inflate in turn from distal to proximal, then deflate, then repeat — closely approximating the natural direction of lymphatic flow from the limb periphery toward the trunk. The Lympha-Press's Pre-Therapy program is a clinical differentiator: before compressing the limbs, the device first stimulates the lymph node territories in the groin, abdomen and chest, clearing space for incoming lymph. Without this step, compressing a congested limb pushes lymph toward already-overloaded nodes — reducing efficacy and potentially increasing discomfort. The Pre-Therapy sequence ensures lymph can flow freely before peripheral compression begins. At DermaEsthetic patients are dressed in the Lympha-Press garment in a reclined position and the session runs for 40–45 minutes unattended or with brief monitoring. The sensation is rhythmic squeezing — pressure that is firm but not painful — and many patients find it deeply relaxing, with a warm flush sensation in the limbs as circulation improves. Post-session the patient may notice increased urination as the mobilised interstitial fluid is filtered through the kidneys; this is a normal and expected response. For primary and secondary lymphoedema, presotherapy is used in combination with manual lymphatic drainage and compression garments as part of Complete Decongestive Therapy. For cellulite and venous insufficiency it is used as a standalone course of ten to twenty sessions, or as an ongoing maintenance protocol. For athletic recovery it is used acutely after intense training to accelerate metabolic waste clearance and reduce delayed-onset muscle soreness. Contraindications include acute deep vein thrombosis, active infection or cellulitis in the treatment area, decompensated cardiac failure, and pregnancy (lower-limb only with caution).
