Scars — whether from surgery, trauma or acne — represent an altered collagen architecture. Normal skin has a basket-weave collagen structure; scar tissue has a parallel, disorganised arrangement that is structurally weaker and cosmetically distinct (different colour, texture, contour from surrounding skin). Fractional laser resurfacing disrupts and remodels this aberrant collagen, partially restoring a more normal architecture over a series of treatments. **What types of scars respond:** Fractional laser is most effective for: - Atrophic acne scars (rolling, boxcar, and to a lesser extent ice-pick types) - Post-surgical scars (after healing is complete — typically at least 6 months post-surgery) - Post-traumatic scars - Stretch marks (striae distensae) — with variable results - Hypertrophic scars — when used in combination with intralesional corticosteroid injections (also available at DermaClinique Písek), producing synergistic scar remodelling **What does NOT respond well:** Keloid scars should be treated primarily with intralesional corticosteroids (available at the clinic); adding fractional laser to a keloid without prior corticosteroid response carries a risk of stimulating further keloid growth. Laser passes are made over the scar and a margin of surrounding normal skin — treating the transition zone is important for blending outcomes. The session is short for small scars (15–30 minutes). Post-treatment, the scar appears redder than usual for 24–72 hours; fine crusting resolves within 4–7 days. **Protocol:** Most scars require 3–6 sessions for clinically meaningful improvement, spaced 4–8 weeks apart. Mature scars (>2 years old) and atrophic acne scars typically need more sessions than early post-surgical scars.
