A three-modality combined protocol for vitiligo using DermaPen microneedling plus PRP, followed by XTRAC 308 nm excimer laser — addressing both the loss of melanocytes and the underlying autoimmune suppression of melanogenesis. The DermaPen creates micro-channels that allow activated PRP growth factors to reach melanocyte progenitors in the hair follicle bulge; the excimer laser then destroys the T-lymphocytes responsible for autoimmune melanocyte killing and stimulates surviving melanocytes to repopulate the depigmented zone. One complete five-week cycle encompasses all three stages; a minimum of five cycles is recommended.

Vitiligo is an autoimmune condition in which T-lymphocytes aberrantly target and destroy melanocytes — the pigment-producing cells of the epidermis — resulting in well-demarcated, depigmented patches. Repigmentation requires two simultaneous achievements: suppression of the autoimmune attack to prevent continued melanocyte destruction, and stimulation of surviving melanocyte progenitors in the hair follicle reservoir to migrate and repopulate the depigmented zone. Derma Medical Clinic's combined vitiligo protocol addresses both requirements in sequence. The first stage is a DermaPen microneedling session over the depigmented patches: the micro-channels created by the needles extend through the epidermis into the dermis, transiently disrupting the barrier that ordinarily restricts the migration of hair follicle–resident melanocyte stem cells. Immediately after needling, activated PRP (platelet-rich plasma) is applied to and driven into the micro-channels. The growth factors in the PRP — particularly VEGF, PDGF and IGF — stimulate melanocyte progenitor activity, improve microcirculation to the perifollicular zone and initiate a mild inflammatory cascade that signals follicle-associated stem cells towards melanocyte differentiation. Following a two-week healing pause, the second stage begins: three weeks of XTRAC 308 nm excimer laser sessions, typically applied three to five times per week. The 308 nm wavelength selectively ablates the activated T-lymphocytes suppressing melanocyte survival in the patches, and simultaneously stimulates surviving epidermal and follicular melanocytes to proliferate and migrate centripetally into the depigmented zone. This completes one five-week cycle. The cycle is repeated; clinical data at Derma Medical Clinic and in the published literature suggest a minimum of five complete cycles for meaningful repigmentation, with response rates varying significantly by patch location — face, trunk and genitalia respond most readily, while hands and feet require more sessions. Session duration for each cycle stage ranges from the DermaPen and PRP session (approximately two hours including numbing and centrifuge time) to the shorter XTRAC sessions (a few minutes per patch). Topical numbing cream is applied before DermaPen.

Preparation Required
