XTRAC 308 nm excimer laser therapy for psoriasis is a targeted ultraviolet treatment that halts the autoimmune inflammatory process driving psoriatic plaques at the specific lesion site, without exposing surrounding healthy skin to UV radiation. The 308 nm wavelength selectively suppresses the hyperactivated T-lymphocytes responsible for the psoriatic inflammatory cascade. Psoriasis is a chronic autoimmune skin condition in which T-lymphocytes abnormally target skin cells, triggering a rapid proliferation cycle that produces the characteristic silver-scaled plaques. The 308 nm excimer laser was developed specifically for psoriasis because this narrow-band UV wavelength is optimally absorbed by the T-lymphocytes driving the inflammatory process. When applied at therapeutic doses, it induces apoptosis (programmed cell death) in the activated T-cells within the plaque, breaking the autoimmune cycle at the lesion level without affecting the uninvolved skin around it — the key advantage over older whole-body UV phototherapy (PUVA or narrow-band UVB cabinets), which necessarily exposes the full skin surface. Derma Medical Clinic introduced the XTRAC excimer laser to Czech clinical practice under MUDr. Michal Sičák, and the clinic continues to be among the most experienced XTRAC operators in the country. The handpiece can be positioned precisely over individual plaques — including those in challenging locations such as the scalp, palms, soles, elbows, knees, nail folds and genitalia — that are difficult or impossible to treat with phototherapy cabinets. A typical session treating a 20% body surface area takes approximately five minutes; sessions feel like a mild, warm sunburn sensation. The standard protocol is one to two sessions per week for a course of ten to twenty sessions, depending on plaque severity, location and skin type. Clinical studies document minimum six-month remission after a complete course; a significant proportion of patients achieve remission of up to two years before re-treatment is needed. The treatment is also effective for plaque recurrence after systemic therapy, and can be combined with topical treatments to reduce their required frequency and quantity.
