Hair loss (alopecia) is driven by several interacting mechanisms depending on its type: androgenetic alopecia involves progressive follicle miniaturisation under dihydrotestosterone influence; alopecia areata is an autoimmune attack on the follicle bulb by T-lymphocytes; telogen effluvium is a stress- or nutritional-triggered shift of follicles from the growth phase into resting; and diffuse thinning can result from scalp inflammation, poor microcirculation or nutrient deficiency at the follicle level. Derma Medical Clinic's combined hair loss protocol addresses several of these simultaneously. The treatment cycle begins with DermaPen scalp microneedling, using fine needles at appropriate depth for the scalp (typically 0.5–1.5 mm) to create controlled micro-trauma across the thinning zones. The micro-injury stimulates blood flow to the perifollicular area, activates growth factors within the dermis, and — critically — disrupts the follicle microenvironment enough to shift follicles from telogen (resting) towards anagen (active growth). The micro-channels also provide direct conduit for the PRP applied immediately after needling: the growth factors — VEGF, PDGF, IGF, TGF-beta — reach the follicle bulge directly, stimulating follicle stem cell activity, improving perifollicular blood vessel density and prolonging the anagen phase. Because PRP is the patient's own plasma, there is no risk of rejection; individual variation in platelet concentration and growth factor composition means that response rates differ between patients, but significant improvement in hair density is well-documented in the clinical literature. Following a two-week recovery period, the XTRAC excimer laser component begins: sessions applied three to five times per week for two weeks deliver 308 nm UV energy to the scalp. This wavelength suppresses the activated T-lymphocytes involved in alopecia areata and scalp inflammation, reduces the DHT-driven inflammatory signal associated with androgenetic follicle miniaturisation, and directly stimulates follicle activity through photobiomodulation. For alopecia areata specifically, XTRAC has the most evidence as a stand-alone modality; combining it with DermaPen + PRP addresses the additional blood supply and growth factor dimensions. Four to five complete cycles are recommended: the first cycle often produces early visible improvement in hair density and reduced shedding; subsequent cycles consolidate the response as follicles stabilise in the anagen phase and the perifollicular environment is progressively repaired.
