Blepharoplasty at Denver Plastic Surgery and Aesthetics addresses the upper and lower eyelids separately or in combination, depending on the patient's anatomy and aesthetic goals. Upper blepharoplasty targets excess skin that accumulates in the upper eyelid with age, causing hooding that can obstruct the peripheral visual field in severe cases and consistently creates a tired, heavy appearance. During upper lid surgery, a carefully planned incision is placed in the natural eyelid crease so the resulting scar is concealed when the eye is open; the excess skin and, if present, bulging orbital fat pads are then excised, and the incision closed with fine sutures. The result is a refreshed, more open upper lid appearance. Lower blepharoplasty addresses the under-eye area, where fat herniation through the orbital septum creates persistent puffiness and bags that are poorly responsive to topical products. The excess fat is either removed or repositioned to smooth the lid-cheek junction and eliminate the shadow-causing bags. A transconjunctival approach — where the incision is made on the inside of the lower lid — leaves no visible external scar and is typically used when no excess skin removal is needed. When skin laxity is also present, a subciliary incision is made just below the lash line. Upper and lower blepharoplasty are frequently performed together as a single session for efficiency, and the procedure is often combined with a brow lift to address brow descent that contributes to upper lid crowding.
