Excess upper eyelid skin (dermatochalasis) is excised through an incision placed in the natural upper lid crease, making the scar virtually invisible in the closed eye. In patients where the ptotic skin genuinely obstructs the superior visual field, upper blepharoplasty qualifies as functional surgery and may be covered by insurance — DPSI's team can advise on visual field testing criteria at consultation. Concurrent brow lift may be added for patients where descended brow position is contributing to the hooding, as correcting one without the other can produce an unsatisfying result. Lower blepharoplasty addresses orbital fat prolapse — the bulging of the three orbital fat compartments through the weakened orbital septum — that creates under-eye bags. Two approaches are available: the transcutaneous approach (incision below the lower lash line) allows direct skin and muscle resection as well as fat removal or redistribution; the transconjunctival approach (incision inside the lower lid) accesses the fat compartments without any external scar, making it ideal for patients with adequate lower lid skin tone who need fat management only. Fat may be redistributed to the tear trough rather than excised, softening the hollow transition from the lower lid to the cheek. Both upper and lower blepharoplasty can be performed as a combined procedure under sedation or general anaesthesia at the AAAASF-accredited Dallas Day Surgery Center. Recovery involves mild bruising and swelling for approximately one to two weeks.
