Mastopexy — breast lift surgery — at Dallas Plastic Surgery Institute is designed for patients experiencing breast ptosis: the sagging and descent of breast tissue and the nipple-areolar complex (NAC) that occurs after pregnancy and breastfeeding, significant weight loss, or as a natural consequence of ageing and gravity. DPSI's board-certified surgeons select the mastopexy technique based on the degree of ptosis and amount of excess skin. A periareolar (crescent or donut) mastopexy is reserved for minimal ptosis, removing a ring of skin around the areola and tightening the areolar border. A vertical (lollipop) mastopexy adds a vertical excision pattern for moderate ptosis, reshaping the breast mound and reducing excess lower-pole skin. A full (anchor or inverted-T) mastopexy is required for significant ptosis with excess horizontal skin, extending the vertical excision into the inframammary crease. The technical objective across all techniques is to reshape the breast into a more projecting, rounded mound and reposition the NAC to sit above the inframammary fold at the point of maximum breast projection. An augmentation mastopexy — combining lift with implant placement or fat grafting — is one of the most frequently requested combined procedures at DPSI and is a technically complex operation that DPSI's experienced multi-surgeon team is well-equipped to perform safely. It is important that augmentation mastopexy candidates understand the tradeoff: adding volume simultaneously with a lift requires careful tension management so that neither the lift incisions nor the augmentation compete for tissue perfusion. Fat grafting to the upper pole during mastopexy addresses deflation with natural-feel volume and is another combination approach offered at DPSI. Recovery is approximately two to three weeks for most daily activities, with six weeks of supportive bra wear and avoidance of strenuous upper-body exercise.
