Lower eyelid blepharoplasty (plastická operace dolních víček) addresses the bulging fat pockets, loose skin and dark circles that develop beneath the eyes with age and, often, genetics. Unlike the upper lid, where excess skin is the main issue, the lower lid problem is usually protruding orbital fat creating bags, sometimes combined with skin laxity and a hollow tear-trough that casts a shadow — together producing a chronically tired or unwell look that no amount of sleep corrects. The surgeon's approach depends on the anatomy. Where the issue is primarily fat, it can often be addressed from inside the eyelid (a transconjunctival route) leaving no external scar, with the fat removed or repositioned to soften the tear-trough hollow. Where there is also excess or crepey skin, a fine incision is made just below the lash line, hidden in the natural crease, to allow conservative skin tightening as well. Patients avoid strenuous activity and protect the eyes from sun during early healing, and the smoother, brighter under-eye contour emerges as swelling resolves. The result is long-lasting.
