Poly-L-lactic acid (PLLA) is a biodegradable synthetic polymer originally used in dissolvable surgical sutures. In its injectable form as a dermal biostimulator, it is prepared as a suspension of microspheres reconstituted in sterile water before injection. These microspheres, once deposited in the subcutaneous tissue or deep dermis, trigger a controlled foreign-body inflammatory reaction that activates fibroblasts in the surrounding tissue. These fibroblasts produce type I collagen — the structural protein responsible for skin firmness and density — filling the treated zone with the patient's own biological collagen rather than a foreign gel. This mechanism distinguishes PLLA from conventional HA fillers in several practically important ways. First, results develop gradually: there is no immediate filling visible the day of injection (the initial volume from the reconstituting fluid resorbs within a few days). The collagen production cascade takes two to three weeks to become visible, and continues to build over several weeks after injection, producing a natural-looking progression rather than an abrupt change. Second, results are substantially longer-lasting than HA fillers: PLLA-induced collagen is not enzymatically degraded in the same way as cross-linked HA; clinical studies and post-market experience document results lasting up to 24 months, with some patients maintaining improvement longer. Third, the volumising effect is the patient's own tissue — it is entirely biocompatible and reversibility via hyaluronidase is not applicable (as there is no HA to dissolve), though the induced collagen naturally remodels over time. At CLED MUDr. Moskalyková administers PLLA for the face — particularly the temples (one of the first areas to hollow with age), cheekbones, chin and jawline — and for body areas where conventional fillers would be impractical due to the volume required: décolletage lines, neck laxity, loose arm skin, buttock contouring, and upper thigh or abdominal skin quality improvement. The injection session itself takes up to 30 minutes. Numbing cream is applied for sensitive patients; the PLLA suspension is administered via multiple small injections or cannula technique depending on the zone. Mild swelling and occasional bruising resolve over a few days. After the first session, the result is typically assessed at three to four weeks, at which point a second session can be scheduled if additional volume is desired. Contraindications include autoimmune disease, active infection and pregnancy.
