Botulotoxin works at the neuromuscular junction by blocking acetylcholine release — the chemical messenger that signals a muscle to contract. When a measured dose is placed in a specific facial muscle such as the frontalis (forehead), corrugators or procerus (glabellar frown complex), or orbicularis oculi (crow's feet), that muscle's contractile activity is temporarily suspended. The overlying skin stops being mechanically creased with every expression; existing dynamic lines soften, and continued treatment prevents the progressive deepening of those lines into static folds. MUDr. Miloslava Moskalyková begins every session by observing the patient's natural facial dynamics — how the forehead lifts, where the brow furrows, how the eye corners crease. This functional mapping determines both injection location and dose per muscle: brow position, brow symmetry and muscle mass all influence how much product achieves the desired relaxation without over-weakening adjacent muscles. Ultra-fine needles are used throughout; the entire injection sequence is typically complete in under ten minutes. Post-treatment protocol at CLED follows established guidelines: patients are advised to stay upright for four hours, avoid strong pressure on the treated areas, skip saunas, vigorous exercise and alcohol for the first six hours, and actively contract the treated muscles in the two hours after injection to assist product distribution. If the lower eyelid is treated, artificial tears are recommended preventively. Effect onset varies between two and twenty-one days — a range driven by individual neuromuscular sensitivity. A follow-up assessment at three to four weeks allows identification of any asymmetry or under-treated zone that may need a small top-up. First-time patients typically experience effects for four to six months; subsequent sessions often extend to six to twelve months as the repeatedly rested muscles require less product to maintain relaxation. Botulotoxin at CLED is also used in the specialist application of hyperhidrosis: a Minor iodine-starch test maps the active sweat gland locations on the underarm, palm or sole before injection, ensuring each injection targets confirmed gland activity. Hyperhidrosis treatment at these sites provides four to twelve months of relief depending on location. Contraindications for all botulotoxin treatments include pregnancy, breastfeeding, neuromuscular diseases (myasthenia gravis, Lambert-Eaton syndrome), use of certain aminoglycoside antibiotics and active skin infection at the planned injection site.
