The Basisdiagnostik Gross profile is the workhorse panel for annual preventive screening. It builds on the Klein profile by adding the clinical detail needed for proper risk stratification across all major organ systems. The full lipid panel — total cholesterol, HDL, LDL, triglycerides, and lipoprotein(a) — enables cardiovascular risk calculation using the European SCORE2 model. Lp(a) deserves special attention: it is a genetically determined, independent cardiovascular risk factor that is elevated (above 50 mg/dL) in approximately 20% of the European population. Unlike LDL, Lp(a) does not respond to diet or statins, but its measurement influences treatment intensity decisions for other modifiable risk factors. A single lifetime measurement is usually sufficient since Lp(a) levels are largely stable. The expanded liver panel adds GGT and bilirubin to the ALAT/ASAT pair from the Klein profile. GGT is particularly sensitive to alcohol-related liver damage and biliary obstruction, while bilirubin elevation can indicate haemolytic conditions, Gilbert syndrome, or hepatobiliary disease. The ASAT:ALAT ratio helps distinguish alcoholic from non-alcoholic liver pathology. Kidney assessment is expanded with urea (blood urea nitrogen), which together with creatinine and eGFR provides a more complete picture of glomerular filtration and hydration status. Electrolytes (sodium, potassium, and potentially calcium and phosphate) screen for disorders of fluid balance and mineralocorticoid function. Albumin serves as a marker of nutritional status and hepatic synthetic function. Low albumin can indicate chronic liver disease, nephrotic syndrome, malnutrition, or chronic inflammation. TSH screens for thyroid dysfunction — both hypo- and hyperthyroidism — which together affect 5-10% of the European adult population and often present with nonspecific symptoms such as fatigue, weight changes, and mood disturbance.
