Lower blepharoplasty addresses under-eye bags and hollowing via two primary approaches: the transconjunctival approach (incision inside the eyelid, no external scar) is appropriate when skin excess is minimal and the primary issue is fat herniation; the subciliary approach (incision just below the lower lash line) allows concurrent skin resurfacing or excision when skin laxity contributes significantly to the under-eye appearance. Fat repositioning — redistributing prolapsed orbital fat into the tear trough — is an alternative to fat excision that avoids the hollowed look sometimes produced by aggressive fat removal. Double eyelid surgery creates or enhances the supratarsal crease in patients with a monolid, available in non-incisional suture techniques for those without significant skin excess and full incisional technique for definitive crease creation. Asian eyelid surgery is approached with cultural and anatomical sensitivity, preserving natural features while addressing the patient's specific goals. Canthopexy and canthoplasty tighten the lateral canthal tendon to support the lower eyelid position, addressing rounding of the outer canthus or lower eyelid laxity. Total surgery time is one to three hours; recovery involves bruising and swelling resolving over ten to fourteen days.
