Blepharoplasty at this practice addresses the three primary periorbital changes associated with ageing: upper eyelid dermatochalasis (redundant skin creating hooding that can narrow the visual field or simply create a fatigued appearance), lower eyelid fat herniation (the bulging of orbital fat through a weakening orbital septum producing visible 'bags' under the eyes), and the combination of both upper and lower changes in patients seeking comprehensive periorbital rejuvenation. Upper blepharoplasty is performed through an incision precisely placed in the natural upper eyelid skin crease, through which excess skin and, where indicated, a strip of orbicularis oculi muscle and herniated fat are removed. The natural lid crease position is re-established and the incision closed to leave a fine scar concealed within the skin fold when the eye is open. For the lower eyelid, Dr. Walton offers the transconjunctival approach — the incision is made on the conjunctival surface of the inner lower lid, leaving no visible external skin scar. Through this access the periorbital fat compartments are repositioned or reduced to eliminate lower lid puffiness. This approach is particularly well-suited to patients with prominent periorbital fat but good lower lid skin tone, avoiding the risk of ectropion (outward rolling of the lower lid) that can occur with transcutaneous (external skin incision) approaches if skin is over-resected. Where lower lid skin laxity coexists with fat herniation, a skin pinch resection or adjunctive chemical peel to the lower lid skin surface may be recommended to address the texural component. Combined upper and lower blepharoplasty is the most comprehensive periorbital rejuvenation option and remains one of the highest satisfaction procedures in facial plastic surgery.
