The sports medicine examination with ergometry extends the basic pre-participation assessment with a bicycle ergometer stress test, which reveals cardiovascular anomalies that only manifest under exertion — including exercise-induced arrhythmias, ST-segment changes indicating ischaemia, hypertensive blood pressure responses, and abnormal heart rate recovery patterns that would be completely invisible on a resting EKG.

The ergometer test follows a stepped protocol in which resistance is increased incrementally while the physician monitors a continuous 12-lead EKG and takes blood pressure readings at each stage.
