The Gold Annual Care Card at CPM Pavla Koláře is the centre's premium coordinated healthcare programme, offering a personal physician with priority scheduling, a dedicated care coordinator, and the full breadth of the clinic's specialist and physiotherapy services. Designed for executives, high-performing athletes, and individuals who want genuinely proactive, personalised, concierge-level healthcare in Prague — combining movement medicine, preventive care, and specialist access under one annual programme.

The Gold Annual Care Card represents CPM Pavla Koláře's most comprehensive package — a full-year concierge care arrangement centred on a personal physician relationship with priority access and a care coordinator who actively manages the patient's care calendar, results follow-up, and specialist referral logistics. The programme includes all the specialist consultation access of the Basic Card and substantially more. In addition to the standard specialist slate, the Gold Card provides access to extended eye examinations and retinal diagnostics (included, not as an add-on), comprehensive skin examination, expanded preventive GP care, and a vaccination consultation and schedule review. The full comprehensive annual check-up — the same programme available standalone at 19,500 Kč — is included within the Gold Card fee. The physiotherapy component is substantially enhanced: 12 individual sessions (60 minutes each) plus 6 massage sessions (60 minutes each) and 6 physiofitness sessions — all included in the fee. Unlimited rehabilitation gym access means the patient can use the DNS-equipped physiofitness gym as frequently as their programme warrants without per-session charges. The care coordinator tracks all of these sessions and ensures the programme is paced appropriately across the year. The personal physician component is what most fundamentally distinguishes the Gold Card: a specific named doctor who knows the patient's history, maintains continuity across every care episode, and is available on a priority basis — bypassing standard appointment waits.
