The programme covers rehabilitation medicine, orthopedics, neurology, gastroenterology, immunology, cardiology, internal medicine, radiology, physiotherapy, electrotherapy, psychiatry, rheumatology, endocrinology, obesity medicine, 12 physiotherapy sessions, group exercise, preventive internal examination, two InBody body composition assessments, basic optometry, and abdominal ultrasound — all within a single annual fee.

The Basic Annual Care Card is CPM Pavla Koláře's entry-level coordinated care programme, designed for individuals who want consistent access to specialist medicine and physiotherapy throughout the year rather than managing each episode of care reactively. The programme fee covers a defined set of specialist consultation slots, physiotherapy sessions, and investigations, all coordinated by the clinic's reception and care team so that the patient does not need to navigate specialist referrals independently. The specialist access included in the basic programme covers rehabilitation medicine (4 visits), orthopedics (4 visits), neurology (4 visits), plus single consultations in gastroenterology, immunology, cardiology, internal medicine, radiology, psychiatry, rheumatology, endocrinology, and obesity medicine — a breadth of coverage that would require managing twelve separate specialist relationships through the standard Czech healthcare system. The 12 physiotherapy sessions (60 minutes each) provide sufficient capacity for managing a typical musculoskeletal episode, maintaining mobility, or progressing rehabilitation from a procedure completed in the year. Two group exercise sessions give access to the DNS-based class programme. Preventive care within the programme includes a full internal medicine examination, two InBody body composition assessments to track changes across the year, basic optometry, and an abdominal ultrasound.
