CT coronary angiography (CCTA) at CCN Amsterdam is an ECG-gated CT study with intravenous contrast to opacify the coronary arteries and allow direct visualisation of their lumen. Unlike the calcium score, CCTA images the full spectrum of plaque morphology — calcified, mixed, and non-calcified (soft) — and quantifies luminal narrowing per vessel segment from the left main trunk to distal branches. The primary indication is stable chest pain or equivalent symptoms with low-to-intermediate pre-test probability of obstructive coronary artery disease, where the 2019 ESC guidelines position CCTA as the preferred first-line test. CCTA has exceptional negative predictive value (>99%) for significant CAD — a normal result safely avoids invasive diagnostic catheterisation. Detected stenoses are graded and triaged toward medical optimisation, stress imaging, or revascularisation assessment as appropriate. Preparation includes: four to six hours of light fasting; avoidance of caffeine for 12 hours; sublingual nitroglycerin just before imaging (routine at CCN); beta-blocker premedication if resting heart rate is elevated; creatinine check if renal function is uncertain. CCN cardiologists review three-dimensional coronary reconstructions with the patient on the day.
