The SMAS (superficial muscular aponeurotic system) facelift is the surgical gold standard for addressing age-related descent of facial tissue — jowling, deepening nasolabial folds, midface volume loss, and neck laxity. By lifting and repositioning the SMAS layer (the muscular fascia beneath the facial skin) as well as trimming excess skin, the surgeon achieves a result that is more three-dimensional and longer-lasting than a skin-only lift, while avoiding the 'pulled' appearance that can result from purely tensioning the skin. BODY klinika's plastic surgeons emphasise that a facelift should preserve natural appearance and facial expression — a key differentiator from techniques focused solely on skin tightening. The incisions follow the hairline and natural facial contours to remain as inconspicuous as possible. Excess skin is carefully resected after the deeper SMAS work, and the skin is re-draped without tension, which minimises scar widening and distortion over time. For patients with neck skin laxity and platysmal banding (the vertical cords visible in the neck), the procedure can include a platysmaplasty to repair the platysma muscle, creating a defined jawline and neck angle.
