Microneedle radiofrequency is a fractional modality that combines two proven skin-remodeling mechanisms: mechanical micro-injury via fine needles and focused thermal energy from radiofrequency current. The combination produces a synergistic biological response — the needle channels trigger the wound-healing cascade (collagen induction), while the RF heat simultaneously stimulates deeper collagen remodeling and tissue contraction. The result is more significant neocollagenesis than either modality alone. At Berska Clinic, this treatment is delivered using the EndyMed Intensif, the dedicated microneedle applicator for the EndyMed Pro platform. Its key engineering features distinguish it from generic microneedle RF handpieces. First, all 25 gold-plated needles are uninsulated along their full length, meaning RF energy is deposited throughout the entire depth of penetration rather than only at the needle tip. This produces a volumetric heating column from the dermis down to the deep dermis at the selected depth, rather than a localised spot at the tip. Second, the Smooth Motion algorithm synchronises needle insertion, dwell time, and RF delivery to minimise mechanical trauma and reduce bruising — a common side effect with older microneedle RF devices. Needle depth is adjustable between 0.5mm (suitable for periocular and perioral areas and thin skin) and 5mm (for deep dermal treatment on the body or areas with thickened fibrous tissue). This depth range covers the full spectrum of dermal indications: superficial fine lines, moderate wrinkles, acne scars, hypertrophic scars, stretch marks, and skin laxity across face, neck, décolletage, and body. Beyond aesthetic indications, the device is also used by MUDr. Běrská for alopecia (scarring and non-scarring) and focal hyperhidrosis — areas where dermal RF stimulation has shown positive evidence in peer-reviewed literature. Procedure protocol: skin is cleansed and a topical anesthetic cream applied, which requires approximately 30–45 minutes of contact time before treatment begins. The active treatment portion takes several tens of minutes (duration varies with area). Patients rest comfortably during the session. Post-procedure, mild erythema and sensitivity typically persist for 24–48 hours. SPF 30 or above is required for a minimum of two months following treatment to protect the remodeling skin from UV exposure. Treatment frequency is determined by indication: acne scarring and skin laxity typically benefit from a series of three to five sessions spaced four to eight weeks apart. Maintenance is individual — some patients return once or twice yearly.
