Prominent ears result from one of three anatomical variations: absence or underdevelopment of the antihelical fold, excessive protrusion of the conchal bowl, or a combination of both. MUDr. Šorma assesses each patient's ear anatomy pre-operatively to determine which correction strategy will produce natural-looking results — the goal is not to flatten ears against the skull but to create an auricular shape consistent with facial proportions, with the ear lying roughly 15–20 mm from the head at its midpoint. The technique employed at the clinic combines high-frequency cartilage scoring — using a precision instrument to reduce the spring of the cartilage along the antihelical fold — with internal permanent sutures that hold the reshaped cartilage in its new position. The incision is made in the postauricular sulcus, the crease directly behind the ear, so the healing scar is entirely hidden. No material is removed from the front of the ear; the reshaping is achieved by modifying and repositioning existing cartilage from behind. Surgery is performed under local anaesthesia on an outpatient basis at the Poliklinika Trnová operating room. Bilateral correction typically takes 60–90 minutes. Immediately after surgery a firm protective bandage is applied around the head, which is worn for approximately two weeks. After bandage removal, patients wear a soft elastic headband — particularly at night — through weeks five and six to protect the cartilage while internal sutures fully consolidate. Contact sports, activities that could cause an ear blow, and exposure to cold should be avoided during this period. Sensitivity in the ear skin may remain elevated for several months as nerves regenerate; this resolves gradually. The procedure is suitable from age six onwards, when the ear has reached roughly 85% of its adult size. Adult patients typically seek correction for ears that have caused self-consciousness throughout life, or for asymmetry following injury. Post-operative photographs taken at the follow-up visits document the achieved result and guide any minor refinements if needed.
