Moles (melanocytic naevi) and other pigmented or raised skin lesions are among the most common reasons patients seek a consultation with a plastic surgeon for cosmetic reasons. While the majority of moles are benign, the decision to remove them surgically rather than by laser has a key advantage: the entire lesion is excised and submitted intact for histopathological examination, providing a definitive diagnosis and ruling out any early atypical changes. The surgical procedure involves injecting local anaesthetic around and beneath the lesion, then excising it with a margin of surrounding normal skin using a scalpel. The wound is closed with fine sutures, with deeper absorbable sutures to reduce surface tension on the scar. For facial lesions, incision placement follows the skin tension lines (Langer's lines) to minimise the visible scar once healed. The procedure takes 20–40 minutes per lesion depending on size and location; multiple lesions can be treated in a single session, with a reduced per-lesion fee for subsequent excisions. Healthcare insurance in the Czech Republic covers mole removal when there is a medical indication — typically provided by a dermatologist who has assessed the lesion and issued a referral documenting concern about its appearance, rapid growth, or other suspicious characteristics. Cosmetically motivated removal of a clearly benign-appearing mole is a private (self-pay) procedure. Patients are advised to consult a dermatologist first if they have any concern about a mole's malignant potential, both to obtain a specialist assessment and, where appropriate, to generate the insurance referral that covers the procedure costs. Post-operatively, the wound is dressed and sutures are removed at five to ten days for facial wounds. Patients are advised to protect the healing scar from sun exposure for at least six months to prevent post-inflammatory hyperpigmentation.
