Enlarged male breast tissue — gynecomastia — arises from one of two distinct pathologies, or a combination of both. Pseudo-gynecomastia, more accurately called lipomastia, involves an accumulation of fatty tissue in the chest without genuine glandular hyperplasia; it responds to liposuction alone. Many patients presenting to MUDr. Šorma have a mixed picture. The pre-operative consultation includes a physical assessment of chest tissue composition — distinguishing glandular from fatty components by palpation — and a review of any possible hormonal causes or medications that may contribute, such as anabolic steroids, some antihypertensives, or marijuana use. Where a hormonal cause is suspected, referral to an endocrinologist for evaluation before surgery is recommended, as the enlargement may recur if the underlying driver is not addressed. For severe gynecomastia involving significant breast ptosis or very large volume — less common but seen in patients with a history of obesity — a more extensive procedure may involve skin excision and repositioning of the areola to a higher position on the chest, creating a longer scar but a dramatically improved contour. All procedures are performed under general anaesthesia with one-day hospitalisation.
