Facial ageing is primarily a story of descent: soft tissue that was once supported by firm subcutaneous compartments gradually loses volume, elasticity, and the ligamentous attachments that anchor it to the underlying skeleton. The result is jowling at the jawline, deepened nasolabial folds, and laxity in the neck. A facelift addresses these changes by directly repositioning the descended tissue — with or without tightening the SMAS (superficial musculoaponeurotic system), the fibromuscular sheet beneath the skin that moves as one with facial expression. The skin-only lift is best suited to patients where skin redundancy is the dominant concern and deeper tissue architecture remains relatively stable; the SMAS lift adds a second layer of correction, elevating and tightening the muscular layer before redressing the skin, producing a result that moves more naturally with expression and typically lasts longer. For patients with excess submental fat or platysmal banding in the neck, both approaches can be combined with neck muscle tightening or fat reduction. The minilifting — also called a limited-incision facelift — places the entire scar in front of the ear only, avoiding the long hairline scar of a traditional facelift. This is the preferred option for patients in their early to mid-forties with early jowling and cheek descent but without significant neck involvement. Recovery from the minilifting is somewhat shorter than a full procedure. Both procedures are performed under analgosedation — a carefully titrated combination of local anaesthesia and intravenous sedation administered by the surgeon that keeps the patient comfortable and cooperative throughout. One-day hospitalisation is included: the patient is admitted the morning of surgery and discharged the following morning, allowing for post-operative monitoring. An elastic compression garment is applied at the end of surgery and worn during the initial recovery phase. Visible bruising and swelling resolve substantially by week three; the final refined result is typically appreciated at three to six months as residual oedema dissipates and the tissue settles. Scars continue to fade over 12–18 months. MUDr. Šorma conducts an extended pre-operative consultation examining facial anatomy, skin quality, and degree of tissue descent to recommend the approach most likely to achieve the patient's goals. For patients also seeking non-surgical adjuncts — such as Dysport (botulinum toxin) for expression lines in the same treatment period — these can be discussed and are billed separately.
