Trichloroacetic acid (TCA) at 35% concentration is classified as a medium-depth chemical peel, meaning it penetrates through the full epidermis and into the papillary dermis. This depth of exfoliation produces more significant clinical results than superficial peels (glycolic, lactic, or low-concentration TCA) while carrying a lower risk profile than deep peels (phenol). TCA works by denaturing proteins in the skin layers, causing controlled chemical coagulation that triggers the wound healing cascade and the production of new, healthier skin. At BeauMED, the TCA 35% peel is performed by an aesthetic medicine practitioner who assesses the patient's skin type, concerns, and history of previous treatments before proceeding. The skin is thoroughly cleansed and degreased to ensure even acid penetration. The TCA solution is then applied in controlled layers using gauze or a brush, with the practitioner observing the skin's frosting response — a whitening of the skin surface that indicates the depth of acid penetration. The number of layers and the degree of frosting are adjusted based on the treatment goals and the patient's skin tolerance. During application, patients feel a burning or stinging sensation that intensifies as the acid penetrates. This discomfort peaks within 2 to 3 minutes and then subsides as the skin's nerve endings are temporarily affected by the acid. Cool compresses or a fan may be used for comfort. The entire application process takes approximately 30 minutes including preparation. The recovery period for a TCA 35% peel is more significant than for superficial peels. Patients should expect redness and tightness on day one, followed by a darkening and bronzing of the skin on days 2 to 3, and visible peeling and flaking from day 3 to day 7. The new skin beneath is smoother, more even in tone, and has improved clarity. Sun protection (SPF 50) is essential for at least 4 weeks post-peel to protect the regenerating skin from UV damage and post-inflammatory hyperpigmentation. A series of 2 to 4 sessions spaced 4 to 6 weeks apart produces cumulative improvement for concerns such as melasma, actinic damage, and textural irregularities.
