Bus/PCV/Taxi Medical

This 30-minute medical assessment is intended for people wishing to drive a bus or passenger-carrying vehicle, and for taxi drivers who require the medical. Beard Medical Practice provides the appointment at Litfield House Medical Centre in Bristol for £95. The service includes the medical assessment for the D4 form and a receipt issued with every medical.

The doctor measures blood pressure, checks eyesight before and after driving glasses are worn, and performs a urine test for glucose. Your current medication is recorded on the D4 form, and your medical history is taken using the questions on that form. The medical section of the D4 form is then completed. Bring a D4 medical form, photo ID, driving licence, driving glasses, a urine sample, current medication details or medication boxes, and relevant hospital letters or consultant details. If you wear glasses or contact lenses, bring a recent glasses prescription or optician’s report. The medical certificate is valid for four months after the doctor signs it. The assessment does not diagnose, treat, prevent, or cure a condition.
