The TSH + fT4 screening pair is the internationally recommended first-line investigation for thyroid dysfunction. TSH (thyrotropin), secreted by the anterior pituitary, is the most sensitive indicator of thyroid status: it rises in hypothyroidism (the pituitary works harder to stimulate an underperforming thyroid) and falls in hyperthyroidism (the pituitary reduces stimulation in response to excess thyroid hormone). fT4 — free thyroxine, the biologically available fraction of the main thyroid hormone — confirms and characterises the functional state: in primary hypothyroidism both TSH is high and fT4 is low; in subclinical hypothyroidism TSH is elevated but fT4 remains normal; in hyperthyroidism TSH is suppressed and fT4 is elevated. Anti-TG (thyroglobulin antibodies) are a secondary autoimmune marker, less specific on their own but informative alongside Anti-TPO. TRAK (TSH receptor antibodies) are the specific marker for Graves' disease and are used both for diagnosis and for monitoring response to antithyroid treatment or assessing risk of neonatal hyperthyroidism in pregnancy. fT3 (free triiodothyronine) — the biologically active form of thyroid hormone — is measured when T3-dominant hyperthyroidism is suspected (TSH suppressed, fT4 normal but fT3 elevated) or in monitoring patients on T3-containing thyroid hormone replacement therapy. Thyroglobulin is measured as a tumour marker in patients who have had thyroid cancer and thyroidectomy, where it serves as the primary monitoring biomarker for recurrence.
