Protruding ears are usually caused by an underdeveloped antihelical fold or an overly deep conchal bowl — both correctable with otoplasty. Through an incision concealed in the crease behind the ear, the surgeon scores and sutures the cartilage to recreate a natural fold and set the ear closer to the head, aiming for symmetric, natural-looking ears rather than an over-corrected, pinned appearance.
