Upper Eyelid Surgery (Blepharoplasty)

Upper eyelid blepharoplasty removes excess skin and fat from the upper eyelids, eliminating the heavy, drooping appearance that accumulates with age and can impair the visual field in advanced cases. At Art Ethic the procedure is most commonly performed under local anaesthesia in about one hour; the surgeon places the incision in the natural eyelid crease so the resulting scar is invisible when the eye is open. Optional adjustments include tear gland repositioning, inner eye corner modification (epicanthoplasty), and Asian eyelid double-fold creation — all discussed at consultation.

Upper eyelid blepharoplasty is one of the most commonly requested facial procedures worldwide because the upper eyelid shows ageing changes relatively early — excess skin begins to accumulate from the fourth decade onwards, creating a heavy, tired appearance and, in more advanced cases, a hooded lid that encroaches on the visual field. Correction restores a more rested, open appearance and can meaningfully improve peripheral vision where true functional impairment exists. At Art Ethic Beauty Clinic, doc. MUDr. Martin Molitor, Ph.D., MBA performs upper blepharoplasty under local anaesthesia for the majority of patients — the eyelid skin is thin and highly amenable to local block, and most patients are comfortable throughout. General anaesthesia is available for patients who prefer it or who are having additional procedures simultaneously. The incision is placed precisely in the natural upper eyelid crease — the line where the lid skin folds when the eye is open. Through this incision, the surgeon removes the planned amount of excess skin, and where orbital fat has prolapsed through the orbital septum (creating a puffy appearance), the fat is removed or repositioned. The incision is closed with fine sutures that leave a scar hidden within the crease when the eye is open and undetectable when the eye is closed after final healing. Optional concurrent modifications include: repositioning of a descended lacrimal (tear) gland to restore the natural upper outer eyelid contour; medial epicanthoplasty (inner corner adjustment) for patients with a pronounced epicanthal fold; and double-fold creation for patients of East Asian heritage who wish to establish an upper eyelid crease. These options are discussed individually at the pre-operative consultation. Recovery involves approximately one week at home with avoidance of screens, reading, and contact lenses during the acute healing phase. Significant activity restriction is not required — walking and light tasks are comfortable within days. Swelling and bruising resolve over one to two weeks; final results are visible at three weeks when residual mild swelling settles. The procedure is not recommended during periods of active infection, recent dental procedures, or significant psychological stress.
