Gynecomastia — male breast enlargement — affects a significant proportion of men, with estimates suggesting it is present in some degree in 30–40% of the male population. It presents in two forms that require different treatments: pseudogynecomastia (fat accumulation in the chest without significant glandular enlargement) and true glandular gynecomastia (hypertrophy of the breast gland itself). Correct diagnosis determines the procedure — liposuction addresses the former, while gland excision is required for the latter. Two cannula options are available: laser-assisted cannulas (which simultaneously tighten the overlying chest skin through the thermal effect) and vibrating cannulas (vibrational liposuction — which removes larger volumes more efficiently and gently). For patients with both fat and skin laxity, the laser option may be preferred. Fat is removed through tiny incisions placed at the inferior areolar border or in the axillary fold — both locations that effectively conceal any residual marks. Volume is measured precisely throughout the procedure to achieve symmetry between both sides of the chest. A compression garment is applied immediately after the procedure and must be worn during the recovery period. Post-operative swelling and bruising resolve over two to three weeks; the final chest contour is visible at eight weeks when all residual oedema has cleared. Patients can return to sedentary work within days; heavy lifting and intense upper-body training resume at eight weeks.
