Buttock lift surgery — gluteoplasty or gluteal dermolipectomy — is a contouring procedure for patients whose primary concern is sagging or descent of the buttock mass, rather than insufficient volume. The most common candidates are patients who have undergone major weight loss (whether surgical or dietary), post-pregnancy patients with skin excess, or those in whom ageing has caused gluteal descent and skin laxity. The approach is selected based on the anatomical pattern of sagging: an incision placed above the upper border of the buttocks (in the lumbosacral crease) enables lifting and resuspension of the descended tissue with the scar concealed beneath waistbands; an incision placed in the infragluteal fold (beneath the buttock) targets lower-pole sagging and is concealed in the natural crease between buttock and thigh. During the operation, excess skin is excised and the remaining tissue is lifted and sutured to higher fixed points — the deep fascia — to provide durable support and prevent recurrence of sagging. Where the patient also has a volume deficit contributing to a flat or deflated appearance, the surgeon can redistribute existing fat during the same session by redraping it rather than grafting new material. Patients who want significant volume augmentation alongside a lift would discuss a combined lift-plus-fat-grafting approach at consultation. Hospital stay is two to three nights for comfort and monitoring. Recovery involves restricted activity for two to three weeks at home; light activity from three to four weeks; unrestricted activity from eight weeks.
