Botulinum toxin anti-wrinkle treatment works by selectively blocking the release of acetylcholine at the junction between the motor nerve and the facial muscle. When a small, precisely calibrated dose is injected into the muscle responsible for a particular movement — such as the corrugator muscle that creates frown lines or the frontalis that lifts the forehead — the muscle relaxes and the overlying skin creases less deeply. The skin does not thicken or change in structure; the lines soften simply because the muscle contracting beneath them is quieter.
