Two surgical techniques are offered. Follicular unit extraction (FUE) harvests individual follicular units (groups of 1–4 hairs as they occur naturally in the scalp) directly from the donor area using a small punch biopsy tool, leaving small circular healing marks that are generally not visible once the hair regrows. FUE is the preferred technique for patients who want to wear their hair short, as it avoids the linear donor scar of FUT. The clinic additionally offers no-shave FUE (also called unshaved FUE), in which the surrounding hair is left at full length and only the individual grafts to be extracted are trimmed, allowing patients to return to social settings without the tell-tale shaved-donor-site appearance that can reveal that a transplant has occurred. Follicular unit transplantation (FUT), also called the strip method, harvests a strip of scalp from the posterior donor zone and the individual follicular units are dissected from the strip under microscopic magnification. FUT allows a larger graft count in a single session than FUE, does produce a linear donor scar (though it is concealed by overlying hair), and follicular transection rates with experienced technicians can be lower than FUE. PRF and PRP concentrates prepared in-house at ARC are applied both to the donor site at closure and to the recipient channel sites at the time of graft placement, with the growth factor scaffold proposed to support graft survival during the critical 24–72 hour post-implantation window when the grafts are avascular and reliant on imbibition for nutrition. Additional post-operative PRF sessions in the weeks following surgery may be recommended. Graft count and technique are determined at consultation based on the degree of hair loss (Norwood/Hamilton scale), donor density, and patient goals.
