For mild ptosis, a periareolar (circumareolar) or crescent mastopexy places a circular incision around the areola to remove a crescent of skin and lift the nipple a modest distance; scarring is confined to the areolar border and fades well due to the colour transition at this site. For moderate ptosis, a vertical (lollipop) mastopexy adds a vertical scar from the bottom of the areola to the inframammary fold, allowing removal of a larger skin envelope and greater nipple elevation. For advanced ptosis or breasts with a large skin envelope following significant weight loss, the inverted-T or anchor scar mastopexy combines the periareolar and vertical components with an additional horizontal incision along the inframammary fold, enabling the most comprehensive skin removal and reshaping. Dr. White uses Alloderm (acellular dermal matrix) or Strattice (porcine dermal matrix) as internal support materials in selected cases — these biologics are sutured to the breast parenchyma and chest wall to create an internal brassiere that supports the lifted tissue and reduces the risk of recurrent ptosis over time. Mastopexy is often combined with implant placement to address both ptosis and loss of upper pole volume simultaneously, though this combined procedure is technically more demanding.
