Dr. Lumiani developed the targeted MRI-guided prostate biopsy technique in 2008, and the team has accumulated extensive experience in prostate MRI interpretation — a skill-dependent modality where radiologist expertise directly affects diagnostic accuracy. The multiparametric MRI protocol uses three complementary imaging sequences on the 3-Tesla scanner. T2-weighted images provide anatomical detail of the prostate gland and surrounding structures. Diffusion-weighted imaging (DWI) measures water molecule movement, which is restricted in densely packed tumour tissue. Dynamic contrast-enhanced (DCE) imaging tracks gadolinium uptake patterns, as tumour neovasculature shows characteristic rapid wash-in and wash-out. Findings are classified using the PI-RADS (Prostate Imaging-Reporting and Data System) scoring system from 1 (clinically significant cancer highly unlikely) to 5 (clinically significant cancer highly likely). ALTA augments the radiologist's assessment with AI-assisted analysis, which may improve consistency and reduce missed findings. The PSA component measures free PSA, complexed PSA, and the free-to-total ratio. The ratio helps distinguish benign prostatic hyperplasia (which tends to produce more free PSA) from malignancy (which tends to produce more complexed PSA). Blood work and inflammation markers provide additional context, as prostatitis can elevate PSA and mimic cancer on imaging. This screening is designed as a non-invasive first step that may reduce unnecessary biopsies by identifying men who genuinely need tissue sampling versus those who can be safely monitored.
