The colon is gently filled with water via a small rectal tube, creating contrast for 3-Tesla MRI imaging of the bowel wall, surrounding lymph nodes, and pelvic organs. No sedation, no bowel preparation, and no radiation exposure.

MRI colonoscopy offers a fundamentally different approach to colorectal screening compared to traditional optical colonoscopy. Rather than inserting an endoscope, the procedure uses a small rectal tube to introduce water into the colon, which distends the bowel and creates natural contrast against the colon wall on MRI sequences. The 3-Tesla scanner captures detailed cross-sectional images of the entire colon, including the caecum and ascending colon which can be technically difficult to reach with traditional colonoscopy. The imaging also visualises structures beyond the bowel wall — adjacent lymph nodes, the pelvic floor, and abdominal organs — providing additional diagnostic information not available through endoscopy. The advantages of MRI colonoscopy include the absence of sedation (patients can drive themselves home immediately), no requirement for bowel preparation (the multi-day laxative protocols that many patients find unpleasant), zero radiation exposure, and a lower complication risk since no instrument enters the colon. The water filling process may cause mild discomfort similar to an enema but is generally well tolerated. The limitations should be understood: MRI colonoscopy may not detect very small polyps (under 6mm) as reliably as optical colonoscopy, and it cannot perform therapeutic interventions such as polyp removal during the same session. If suspicious findings are identified, a follow-up optical colonoscopy with biopsy capability may be recommended. The procedure is particularly suitable for patients who have avoided traditional colonoscopy due to anxiety, sedation concerns, or difficulty with bowel preparation, while still wanting colorectal cancer screening. The session lasts approximately two hours including preparation, imaging, and post-scan review.
