Profhilo at Skinity Clinic is the highest-concentration HA bioremodelling injectable currently available in Europe — a 64 mg/2 mL formulation of thermally stabilised hyaluronic acid that is not cross-linked in the traditional filler sense. Rather than creating volume or projection, Profhilo disperses through the tissue hydrating, tightening, and remodelling the skin architecture at the cellular level. MUDr. Martin Bortlík administers it using the BAP (bio-aesthetic points) protocol — five anatomically defined injection points per cheek — that distribute the product through the tissue's natural hydraulic pressure. The result is improved skin quality, elasticity, firmness, and a luminous skin tone that emerges progressively over 4–8 weeks as the four types of collagen and elastin stimulation take effect. Profhilo (IBSA Farmaceutici Italia) is classified as a bioremodeller rather than a filler, reflecting its fundamentally different mechanism of action. Standard HA dermal fillers use chemical cross-linking (BDDE) to create a stable gel that occupies volume at the injection site. Profhilo, by contrast, uses a thermal NAHYCO® hybridisation process to create a stabilised hybrid cooperative complex of high- and low-molecular-weight HA without cross-linking agents. This makes the product highly biocompatible and allows it to flow through tissue — hydrating the entire compartment rather than creating a localised depot. **Mechanism of action:** Once injected, the HA complex exerts osmotic hydration of the surrounding extracellular matrix and stimulates four key types of collagen (I, III, IV, VII) and elastin production via fibroblast activation. Unlike conventional fillers that primarily occupy space, Profhilo improves the quality of the tissue it moves through — practitioners describe it as 'remodelling the scaffolding' rather than 'filling a cavity.' **The BAP (Bio-Aesthetic Points) injection protocol:** Profhilo is injected into 5 anatomically defined superficial subcutaneous points per side of the face — points chosen to exploit the face's natural hydraulic pressure gradients, allowing the product to distribute evenly throughout the mid-face, lower face, and periorbital zone from just 10 injection points.
